Mountain Trifecta Alliance

Our nonprofit was born out of personal experience and a profound understanding of the challenges faced by individuals with chronic, often invisible illnesses such as Ehlers-Danlos Syndrome (EDS), Postural Orthostatic Tachycardia Syndrome (POTS), and Mast Cell Activation Syndrome (MCAS). These conditions can be debilitating,isolating, and misunderstood. Recognizing the gaps in resources, financial support, and comprehensive care available to people affected by these illnesses, we saw need for an organization that could bridge these divides and provide the support that is often missing.The idea began with a few simple but powerful goals: to build a community,provide reliable resources,and educate both patients and healthcare providers. As we shared our vision with others, it quickly became evident that there was a strong demand for a supportive network specifically tailored to these conditions. With the dedication of our founders and the backing of early supporters, we began the groundwork for establishing an official nonprofit structure to bring these goals to life. While we are in the early stages of our journey, we are focused on building a foundation that will allow us to serve our community effectively for years to come.Our journey is just beginning but our commitment is unwavering. We look forward to growing alongside our community, increasing our reach, and making a meaningful impact in the lives of those who need it most.

We are breaking down barriers to healthcare by offering financial assistance, resources, and connections to specialized care. Our support ensures that individuals with chronic and complex conditions can access essential medical treatments, therapies, and medications that might otherwise be financially or logistically out of reach.

Education and Awareness

One of the main pillars of our impact is raising awareness and educating both healthcare providers and the community about the challenges faced by individuals with EDS, POTS, and MCAS. Through workshops, seminars, and educational materials, we help improve the understanding of these conditions, reducing stigma and encouraging more compassionate, informed care.

Support Networks and Community Building

Our support groups, online forums, and social events create a sense of community and belonging for individuals who often feel isolated due to their invisible illnesses. This network offers emotional support practical advice, and shared experiences that empower individuals to cope with their conditions while building lasting relationships with others who understand their struggles.

Through our financial assistance programs, we alleviate some of these burdens, helping individuals stay financially stable and focused on their health a well-being rather than the stress of financial insecurity

1. Referral Process Overview; 

  • Referrals may be made by healthcare providers, patients themselves, or family members. The referral process includes verifying eligibility,reviewing needs, and connecting individuals with the appropriate services, including financial assistance if necessary.

  • 2. Eligibility Criteria

    • Patients: To be eligible for services, patients must meet the following criteria:

      • Diagnosed with EDS, POTS, MCAS, or other chronic illnesses.

      • Demonstrate a need for services (e.g., patient education, support groups, or financial assistance).

      • Meet financial criteria for assistance based on the 2025 federal poverty guidelines.

    • Healthcare Providers:

      • Healthcare providers must meet the following criteria:

        • Medical professionals or clinics serving patients with the Trifecta.

        • Interested in subscribing to Mountain Trifecta Alliance for patient support or utilizing one-time in-office support services.

  • 3. Referral Process

    • Step 1: Referral Submission:

      • Patients or healthcare providers should submit a completed referral form, either online or through direct contact with the referral coordinator. The form should include:

        • Patient contact information and diagnosis.

        • Household income and family size for financial assistance assessment (if applicable).

        • Services requested (e.g., financial assistance, educational resources, support groups).

      • For healthcare providers, details about the practice and desired service (monthly subscription or one-time support).

    • Step 2: Review of Referral:

      • Upon receipt of the referral, Mountain Trifecta Alliance will review the patient’s information to determine eligibility based on financial status and specific needs. For healthcare providers, the referral will be reviewed to assess the need for monthly subscription or one-time support services.

    • Step 3: Service Assignment:

      • Based on the review, the patient will be assigned to the appropriate service (financial assistance, education, support groups, etc.).

      • Healthcare providers will be enrolled in the service plan that fits their needs (monthly subscription or one-time session).

    • 4. Follow-Up and Support

      • Step 1: Confirmation:

        • Patients and healthcare providers will receive a confirmation of their referral status, including the services they are eligible to receive.

      • Step 2: Ongoing Communication:Mountain Trifecta Alliance will maintain regular contact with the referred parties, ensuring services are being utilized and meeting the patient’s needs.

      • Step 3: Feedback & Evaluation:A satisfaction survey will be distributed after 3 months of service to assess the effectiveness of the referral and any areas for improvement
